$45-50K Annually

Mon-Fri; 8am-4pm;9am-5pm;10am-6pm

JOB SUMMARY:         

Primary responsibilities will be to support the Family Services and Disability/Mental Health content areas. This will include, but is not limited to, collaboration work with staff members and community partners to ensure appropriate services to families and children. Promotes strength base skills to help families develop leadership skills, build confident in their work and daily lives, ability to access and utilize resources, and encourages ideas for higher educational attainment for families and children.

Responsibilities:

I.                    Disabilities Service

·         Provide assistance and support for the Family Services content areas at all Early Learn sites. Make sure IEP mandates are complied with by following up with the districts and therapeutic agencies which includes the monitoring of deliverance of services

·         Track children with disabilities from birth to grade 2. Lead Facilitator for the Special needs Parent Alumni

·         Integrated Classroom liaison between Grand Street Settlement and NYCCD. Advocate for parents and conduct tours for potential families. Complete registration packet 

·         Liaison with community partners for possible new partnerships and maintain on-going positive relationship with partners to enrich the program with services to meet the needs and goals of families and children

 

II . Family Services

·         Works closely with Family and Community Partnership Coordinator; assisting with governance, PIR, Community Assessment, Roll over, and other grant reports

·         Ensure eligibility, recruitment; selection, enrollment, and attendance are in compliance according to ACS/HHS for placement process

·         Work collaboratively with Family Worker on caseloads to help families develop family partnership agreement and goals

·         Document all work done with families in the family services binders, including but not limited to: workshops, events, and meetings

·         Assist with inputting information into the COPA system

·         Assist with other aspect of the program as needed to ensure high quality services to families and children

·         Work with partnering schools to help with Kindergarten enrollment as an Advisor to parents and maintain Kindergarten Advisor Tracking sheet

Qualifications:

·         Bachelor’s Degree in Social Work or a related field required

·         Must have excellent communication skills and the ability to set appropriate priorities to met deadlines

·         Training and experience in case management, community advocate, working with community based organizations, and/or social work are highly desirable 

·         Knowledge of social services framework, proficiency with Microsoft Office programs, and computer savvy

·         Some evening work hours required  

Application Instructions:

·         Basic computer literacy in email word processing and internet navigation

·         Clearance through DOE Fingerprint Screening, NYS Central Registry, Sex Offender Registry clearance, 3 reference checks, physical examination with updated TB test, varicella, tdap, MMR (fees may apply)

·         Obtain Mandated Reporter training certificate (every 2 years) and comply with agency’s policy and procedure regarding identification and reporting of child abuse and neglect

·         Obtain First Aid/CPR and Preventive Infectious Diseases certificate within the first year of employment

·         Meet all employee health requirements in Article 47 (DOHMH) and Head Start Performance
